Dermoscopy and the Early Detection of Skin Cancer

A dermatoscope, as seen above, is a handheld device that aids significantly in the early detection of skin cancer. Not only does it magnify the objects in view by ten, but the special optics and LED lights also allow the examiner to see through the surface layers of the skin into the upper layers of the dermis. It is within these layers of the skin where abnormalities can be appreciated that are unable to be seen with the unaided eye or even with a typical magnifying glass.

Dermoscopy, or Epiluminescent Microscopy (ELM), is more widely used as a diagnostic tool in European countries, but it is gradually becoming more popular in the United States. Increasingly more scientific studies are giving credence to its efficacy when used by a trained physician. In addition to providing more accurate diagnoses of both benign and malignant growths, ELM is also able to detect malignancies in earlier stages, which translates into fewer biopsies. In other words, a physician trained in the use of ELM can more clearly distinguish those things that are without a doubt benign, for which no biopsy is needed, from those things that are clearly malignant, for which treatment is necessary. ELM is an integral tool because it helps a physician see subtleties of skin cancer that may not be so clearly defined without it.

The vast majority of melanomas detected with the aid of this device are in a very superficial stage and therefore less likely to be as potentially dangerous when treated this early. As far as the more common types of skin cancers, known as basal cell and squamous cell carcinomas, ELM often allows physicians to detect these at a stage where they may be difficult to see with the naked eye. Therefore, the necessary treatment can be tailor-made and less extensive, resulting in very high cure rates, a more affordable cost, and a less intrusive procedure for each patient.

In my hands, dermoscopy has become a valuable tool in helping me distinguish early pre-cancerous spots from early cancers. This means that I can treat some spots or lesions that are more clearly benign or only in the pre-cancerous stage with more conservative and less costly means. Most importantly, ELM helps me treat skin cancers with the highest degree of accuracy and confidence, which spells peace of mind for me and my patients.
